Adolescent Community Reinforcement Approach (A-CRA)

A-CRA is a community-based treatment for youth and young adults to support recovery from a substance use disorder. The approach is strengths-based, client-centered, flexible, and prioritizes skill building with individuals and members of the family.

A-CRA was developed by Chestnut Health Systems and adapted from the Community Reinforcement Approach for adults to meet the specific needs of youth and young adults with substance use disorders; including those with co-occurring mental health disorders who were not getting their substance use treatment needs met in traditional, clinic-based, mental health programs.  A-CRA is a brief (12-14 sessions), intensive, evidence-based treatment that engages clients by bringing services to them in their home and community, i.e. in settings in which they are frequently more comfortable.  In Massachusetts, CHD is one of a few agencies with Masters Level clinicians trained and certified to deliver A-CRA.  The model capitalizes on activities and goals established by the client and emphasizes collateral coordination with other important systems, including education, juvenile justice, child welfare, and mental health.

A-CRA involves parents, primary caregivers, or other significant adults in a young person’s life in sessions alone and together with the youth. These sessions are designed to foster better communication and to support parents / caregivers in being positive role models to aid the youth in their recovery from a substance use disorder.

Population Served:  Substance use or dual-diagnosed clients ages 13-17, and their families

Program Goals:

  • Help adolescents life a life free from alcohol, marijuana, and other drugs
  • Help adolescents experience positive social activity
  • Promote positive peer relationships and improved relationships with family
  • Help the adolescent and caregiver(s) create a home and community environment conducive to recovery
  • Provide adolescents problem-solving, communication, and other important skills to help maintain sobriety
