The goal of Community Based Flexible Supports (CBFS) is to support adults with chronic and persistent mental illness to be successful in the community. At CBFS, we work with each individual to find their highest level of functioning. For some people, this may mean living in one of our group homes or breaking the cycle of psychiatric hospital admissions. Other people will live on their own with a level of support that allows them to be independent. Still others will reach a point where they no longer need to receive services. All of this is accomplished with a focus on community based work – and on creating strong therapeutic relationships. 80% of our work is with clients in their communities. We assist them is all aspects of daily living while also providing clinical support in our Holyoke and Springfield, Massachusetts locations. Flexibility is key to CBFS. We support people with their employment, school, leisure activities, shopping, medication – whatever they need to live the kind of life they want to lead.
In addition to community based outreach services, CBFS provides groups focused on helping people develop coping skills, overcome substance abuse issues, trauma and other challenges. Specific men’s and women’s groups are offered along with a variety of other topics. Wellness activities and groups are also offered in conjunction with CHD’s Health and Wellness Program. Individual therapy and other counseling services are also available through CHD’s Outpatient Behavioral Health Clinics.
CBFS offers a variety of social activities as well. Having friends, a sense of belonging, celebrating holidays and doing things you like. These are things that are important to all people. Every summer CBFS participants have the opportunity to go to camp and we have events like a Memorial Day Cook Out, Christmas dinner, Holiday Party and talent show. We are here for people 365 days of the year. We also accompany people on trips or to major family events they wouldn’t be able to attend on their own.
