Life can feel overwhelming for anyone, especially for those managing mental health and recovery needs. Our CSS workers can help you coordinate care and assist with more than you think.
CSS includes:
- Community Support Program – Designed for people whose mental health issues are interfering with their ability to access healthcare services, this program helps participants live independently in the community and avoid unnecessary hospitalizations.
- Recovery Coaching & Navigation – Recovery coaches bring lived experience to the picture and help others going through treatment. These caring mentors assist with everything from finding resources and handling everyday responsibilities.
Participants also receive VIP access to our team nurse along with a wide range of CHD wellness programs and services.
